AWE very near miss from Co-pilots point of view

Hi all. Just found this over on airliners.net and thought some of you might be interested. Very well written account of a near-miss at LAS between an America West A320 and an Air Canada A320 - Was entirely blamed on the controller and he was "relieved of his duties". It's written by the co-pilot of the America West that barely missed the Air Canada. Very good read.

The Air canada flight wasn't given a 'hold short of runway 25R, remain this frequency', which definitely was required. Definitely a deal/operational error on the controller's part (though the FO's account doesn't mention if he had heard them give the hold short call) but, definitely an error on the pilot's side as well.

I don't remember a time at all when an additional instruction for holding short or crossing 25R wasn't given as soon as an arrival exited 25L.
