OK. So like almost all crashes this was a "perfect storm" of things going wrong. The pilots not "flying" the plane.
Over-reliance on automation. Complicated or counter-intuitive automation. Cockpit protocol - perhaps made worse
by cultural traditions. So who, including ATC, could have stepped up, or stepped in, and said "Hey, we have a serious
problem here". How can two professional pilots fly a state-of-the-art airliner into the ground ?
