LAX Flight check go-around
« on: June 21, 2012, 04:06:11 AM »
At about 0056-0057 local heard a flight check try to do a low approach on 24L with a A380 on the runway.  Tower issues an immediate go around, pretty interesting.

Hear back read back errors..  Flight check calls in on ILS 24L low approach, tower clears for low approach to 24R. Flight check does not read back runway.  Tower puts a380 on 24L and notices flight check is lined up on the left, sends around. What we have hear is a failure to communicate.

Right after the tower clears the A380 to line up and wait on 24L, I can't understand the beginning of the next transmission, but it ends with "24L standing by for runway check".  Could that be the A380 saying he was going to continue to hold short of 24L waiting for the runway check? Seems reasonable that he could have picked up the confusion if he had been on tower freq long enough...

I didn't pick up on much confusion until the controller noticed the Flight Check (FLC) was lined up for the left. I think the QANTAS (QFA) was letting the controller know they weren't going to be ready to take off till a runway check (fod check?) was complete. If the QFA was still holding short there wouldn't have been a need to send FLC around. Though it's possible the controller sent FLC around with QFA holding sort to be safe. I have a feeling QFA was taking the runway since the controller was quite adamant about FLC offsetting to the right on the go around.

Yeah, that's the scary part - nobody was confused!  Despite the fact that FLC consistently says 24L and tower consistently says 24R, and neither corrected the other. Not sure about the weather at the time, or the tower's view of the QFA flight, but in the controller's mind he had an aircraft holding on 24L and a low approach on 24R. When he realized the low approach was actually on 24L, he immediately sent them around. Since he had already missed evey one of FLC's 24R readbacks, he may have also missed QFA's call that they were not moving into position yet, if in fact that is what they said.

Negative, call was ops 120 an airport operations vehicle on taxiway victor waiting to do a runway sweep, I'm thinking it's required after A380 departures due to outboard engines possibly blowing fod onto the runway.

Qfa does line up and wait which is the reason for the early go around and offset to the right. aircraft can't overfly other aircraft in position.

Right at 0:57, tower has just said "There's an airplane on the runway", and I think keeps talking, but Flight Check then keys up over tower and says "OK we're going around ... and we're offsetting to the North."

That's why tower is a little more urgent in his next "Go around ... offset..." instruction, because he didn't get the call from FC that they had already initiated that.

Lesson:  even when it's urgent, you have to wait until the frequency is clear, otherwise you are wasting a transmission - and time.
