Title: Japan airlines flight 1 take off without clearance feb-23-2009?!

Hi,
I was listening to KSFO Tower on Feb 23 live, and I thought I heard JAL1 take off after TO clearance was canceled.  How bad is this violation (if it is one)?  What might happen to the crew?

If you start listening from about 25 minutes into the clip, you will hear "Japan 1".
When I was listening to this live, I heard Japan 1 saying "going to departure, good day" which is probably in the 1:00pm to 1:30PM PST, and this clip is from 12:30PM to 1:00PM.

You'll find the JAL1 stuff at 28:15

It sounds like there were a host of radio problems, but unfortunately we can't pick up the transmissions sent from the ground, so we don't know what JAL1 ever replied.
